To register your team, first carefully read the Team Requirements and Fundraising & Pledging information.

Next, download and print the Team Registration Form. Complete all fields and submit the form via fax, mail or in person to the contact information listed at the bottom of the form and at the bottom of this page. Be sure to clearly print your Team Captain’s contact information, as we will be contacting the Captain to set up your team’s online fundraising page.

Finally, take note that prior to participation in the Get on Your Bike! event, all team members must complete and sign both the Medical Details Form and the Waiver & Release Form. In addition, any team members who will be underage as of November 29, 2009 must have a parent or legal guardian complete and sign the Minor Consent Form before participating in this event. The completed forms can be submitted along with the Team Registration Form, returned to us by fax, mail or in person prior to the event, or submitted at the check-in desk during the event on November 29, 2009.
